About the Healthy Communities Assessment
Prepared for the California Department of Public Health
The California Obesity Prevention Plan Healthy Communities Assessment Report summarizes the results of the California Department of Public Health assessment of obesity prevention efforts among community collaboratives, surveyed in August-September, 2011.
This on-line survey was conducted by California Convergence.
The survey evaluated diverse activities of current and potential members of the California Convergence network of collaboratives. The survey reached out to more than 300 community advocates statewide, with responses from 112 CAC members, and 80 complete survey responses.

The survey was powered by Community Commons, as an in-kind contribution from the IP3 nonprofit (co-founded by the Center for applied Research and Environmental Systems – CARES – and Community Initiatives).
The Community Commons site includes a newly populated interactive map, including 80 California Convergence member profiles. The Community Commons interactive mapping system provides data and contact information on place-based, multi-sector, equity-focused policy and environmental change initiatives in California and across the U.S.
This innovative web-based tool is a dynamic system that can provide tools for community collaboratives to share information and create a collective voice for change.
